Please provide your details for inclusion on your bed donor acknowledgement plaque, recognising your donation. We'll be in touch with your artwork proof for approval, prior to production and will let you know when your plaque has been installed.
Artwork guide
- Your inspirational quote should be max 100 characters, incl spacing.
- Please provide a quote author if applicable; alternatively, please indicate if your message is an original/personal message.
- You have the option to supply a logo or name (in text) representing the bed donor.
- Logo should be in png, jpg or eps format; positive version for white background.
